VOLUNTEER FORM

Volunteers must have a volunteer application on file with HHC – Refer to ‘Volunteer Application’ section to fill in a volunteer application.  Medical professional must have credentialing forms on file before they sign up to volunteer.  The non clinical volunteers need to have their forms submitted and also to attend an orientation at HHC which is held twice a month.  Please refer to the Volunteer Application section for more information.
