In order to assist Harmony Health Clinic in providing our patients with the best quality health care available it is necessary for all eligibility and medical intake forms to be thoroughly completed and brought with you to the clinic at the time of your appointment. If you have difficulty filling out any of the forms, or speak Spanish and require assistance with translation, the Clinic has volunteers that can provide help on the day of your appointment. Should you require such assistance, please allow for the extra time required by arriving early.
